• All Investigators must complete Travel Supplemental Disclosure Form (TSDF) for
  • any reimbursed travel or
  • any sponsored travel
    • i.e. paid on behalf of the Investigator and not reimbursed to the investigator so that the exact monetary value may not be readily available
• One form per travel event
• Disclosure must occur within 30 days of reimbursement or travel event if sponsored travel
