- Non-OUHSC employees, including all OMRF members, seeking a renewal appointment for the first time since December 2009.
This application should be completed by a graduate faculty member
seeking to renew a primary or secondary graduate faculty appointment.
This application should be completed by a graduate faculty
member seeking an appointment in a graduate program other than their primary graduate
program for the first time.
- A temporary appointment may be granted to a person who by reason of professional expertise or special qualifications can be a contributor to a specific project, thesis, or dissertation committee or who will be teaching on a temporary basis in a graduate program.
- This application must be entered by the departmental chairperson or liaison.
- The request for a temporary appointment requires (1) current CV or biosketch, (2) contact information, and (3) proposed roles of the person who would have a temporary appointment. If the temporary appointment is for a student’s committee, please provide the name of the student, type of committee (e.g., General Exam or Dissertation Committee), and names of other committee members.
