New Appointment

This application should be completed by:

1. OUHSC faculty members and non-OUHSC faculty members seeking a graduate faculty appointment for the first time.

2. Non-OUHSC employees, including all OMRF members, seeking a renewal appointment for the first time since December 2009.
Renewal Appointment
This application should be completed by a graduate faculty member seeking to renew a primary or secondary graduate faculty appointment.
Secondary Appointment
This application should be completed by a graduate faculty member seeking an appointment in a graduate program other than their primary graduate program for the first time.
Temporary Appointment
1. A temporary appointment may be granted to a person who by reason of professional expertise or special qualifications can be a contributor to a specific project, thesis, or dissertation committee or who will be teaching on a temporary basis in a graduate program.
