This application should be completed by OUHSC faculty members and non-OUHSC employees
seeking a graduate faculty appointment for the first time.
Procedure for Applying:
1. Complete the Create User Profile
- OUHSC faculty members will use their OUHSC User ID to complete the Create User Profile
- After creation of the User Profile, non-OUHSC employees will be emailed a User ID
and Password to the email address provided.
2. Log In
- OUHSC faculty members can log in immediately after submitting the Create User Profile
by using their OUHSC ID and Password.
- Non-OUHSC employees or OUHSC faculty members wishing to complete the application
at a later date can log in by selecting the blue Log In tab at the top of this page
and use their OUHSC provided ID and Password to access the Applicant Home Page.
Select ‘Create a new application’ at the top of the page.
3. Complete and submit the application
This application should be completed by a graduate faculty member seeking to renew
their graduate faculty appointment.
Procedure for Renewing:
1. Log In
- Select ‘Log In’ from the blue heading bar.
- OUHSC faculty: Insert campus User ID and Password.
- Non-OUHSC faculty: Insert a User ID (primary institution email) and the password
created when you created the user profile for your original application. If you
cannot remember the password, insert any password. If the database rejects the password,
click the link ‘Forgot the Password’. A default password will be sent to you. For
security reasons, change the default password to a new password.
2. At the Applicant Home Page click the ‘Renew’ link at the right of your application
list.
3. A pre-populated application will appear. Update the information in the application
and attach a new biosketch prior to submitting your application.
This application should be completed by a graduate faculty member seeking an appointment
in a department outside their primary department.
Procedure for Applying:
1. Log In
- Select ‘Log In’ from the blue heading bar.
- OUHSC faculty: Insert campus User ID and Password.
- Non-OUHSC faculty: Insert a User ID (primary institution email) and the password
created when you created the user profile for your original application. If you
cannot remember the password, insert any password. If the database rejects the password,

2. At the Applicant Home Page click ‘Create a new application’ at the top left side
of the page.
3. A pre-populated application will appear with information from the primary application.
Check the application for accuracy. Scroll to ‘Application Type’ near bottom of
page and confirm secondary box has been checked. Select the name of the secondary
graduate program you are applying to, select the appropriate academic rank, attach
biosketch, and submit.
A temporary appointment may be granted to a person who by reason of professional
expertise or special qualifications can be a contributor to a specific project,
thesis, or dissertation committee or who will be teaching on a temporary basis in
a graduate program.
Procedure for Applying:
1. Department Chair or Liaison should select ‘Log In’ from the blue heading bar
and log in using campus User ID and Password.
2. Select ‘Log in as Graduate Faculty Department Chair or Liaison’.
3. Select ‘New Temporary Appointment’ from blue menu bar at top of page.
4. Complete and submit application.
Please note:
- Once application information entry has begun, all information must be complete for
successful submission. It is not possible to re-enter the application to
complete data entry at a later date.
- Information entry may not be inactive longer than 60 minutes. After 60 minutes,
security will time out the process; information must be resubmitted.
Graduate Faculty Appointment Committee
The Graduate Faculty Appointment Committee may access this tab and use the Log In
option on the top, right hand section of the page to access their page of options
for reviewing, making recommendations on applictions, etc.
Department Chair or Liaison
The Department Chair or Liaison may access this tab and use the Log In option on
the top, right hand section of the page to access their page of options for reviewing,
making decision on appliactions, creating temporary appointment applications, etc.
