Prior to filling out this evaluation, check with your supervisor to find the following information:

• if you need a respirator
• if you will be handling volatile gases
• if you will be handling human body fluids or tissue cell lines

Also check your immunization records to find the date(s) of your most recent Td or TDAP vaccine and completion of the Hepatitis B vaccine series. If you plan to work with non-human primates, you will also need to find the date(s) of your most recent TB screening and COVID vaccine series.
